Social Prescribers

Our Social Prescribers are on hand to deal with anything that is not clinical. They cover a very vast array of problems both social and practical.

Social Prescribing is a service provided by non-healthcare professionals called link workers. Link workers aim to support individuals with a range of social, emotional, and practical needs through referrals to non-clinical services in the community. These services can include day centres, charities, volunteers and community groups.

The following list is not exhaustive, but gives an idea of the kinds of things that they deal with:

  • Helping with bills or disputes over bills
  • Help with getting around the home, referring for assessment
  • Help and advice re loneliness and arranging social events
  • Help with Shopping or Collecting Prescriptions
  • Putting you in contact with the correct services ie Citizens Advice or Carers association
  • Arranging lifts for appointments
  • Advising on financial matters
  • Help with low level mental health, low mood and anxiety
  • Help with bereavement
  • Help with financial arrangements via Social Services
  • Help with assessments around your home for grab rails etc.

Social prescribing is a great way to get involved in the community and participate in exciting activities that not only improve your health physically but also mentally too.
