If you are planning to travel abroad and think that you may require vaccinations you need to collect a Travel Questionnaire form from the practice. Once completed this form should be dropped in at the surgery as soon as possible.
Our Practice Nurse will then assess which vaccinations, if any, you require. Following this assessment, you will be contacted for an appointment to be made with the nurse.
Travel Vaccines
- Please remember you must allow sufficient time as some vaccinations and medications have to be given several weeks prior to the date of travel so the sooner you let us know your travel destination and fill in the form, the more likely you are to fit in all of the vaccinations that you are required to have before travelling.
