How to Register With Us

Firstly, please check you live within our practice area; you can do this using the map below

If you are eligible to register with the practice you can register with us by:

  • Visiting the practice to obtain the necessary forms
  • Calling the practice on 01606 544744

Catchment Area

In accordance with the Accessible Information Standard, please advise us of any special needs and your preferred method of contact if required ie Braille, large print, telephone, email etc. These can be found on the New Patient Questionnaire.

We will contact you via your preferred method of contact to advise you of any checks that you are due with regards to your healthcare. We will also contact you to advise of any vaccinations or procedures that you are eligible for, or if the GP or other healthcare professional wishes to see you or wishes us to contact you.
Every patient has a named accountable GP. This information will be text to you when you register at the surgery. However, as a practice, the GP’s will see any patients who are registered at the surgery and you can see any GP of your choice.

Find Your NHS Number

Sometimes when filling in online forms or speaking with our staff, you may be asked for your NHS number.

This online tool will help you find your number if you do not know it.

Please Note

You do not need to know your NHS number to use NHS services, but it can be useful to have it.

Other ways to get your NHS number

If you cannot get your NHS number online you can:

  • find it on any letter from the NHS like a prescription or appointment letter
  • call your GP surgery and ask them for your number

Temporary Patient Registrations

If you are not registered with a doctor but need to see one, you can receive emergency treatment for 14 days. After 14 days you will need to register as a temporary or permanent patient.

To register as a temporary resident you will need to contact the surgery and complete and return the ‘Temporary Patient Registration’ form.

You can register as a temporary resident with a GP in England if you are in the area for longer than 24 hours, but less than three months.
